How much does Zepbound cost in 2026?
There is no single Zepbound price. Start with three details: the exact presentation on your prescription, your insurance coverage, and the program you qualify for. A list price, a pharmacy quote and a savings-program price answer different questions.

Why the starting cash price is not a refill quote
Other prescribed strengths have different self-pay prices. The Journey offer for designated KwikPen or single-dose vial presentations lists $449 per 28-day supply; continuing eligibility requires purchasing the refill within 45 days of receiving the preceding fill. Otherwise, regular prices apply. Injection supplies are additional purchases; taxes and fees may apply under the Lilly savings terms. A program's refill window is an eligibility condition, not a treatment schedule. Ask your prescriber about the clinical plan and your pharmacist about the prescription's timing. Do not change treatment to preserve a discount.
What does “with insurance” actually mean?
A useful insurance quote answers more than “is it covered?” Ask your plan and pharmacy to identify:
- The exact covered product and presentation.
- Whether the prescription's indication meets the plan's criteria.
- Whether prior authorization has been approved, is pending or is required.
- The amount you owe for this fill after the deductible and any accepted assistance.
- How the estimate changes for a different supply period or a future fill.
Keep the written answer with its date. An estimate for a different pen, a different plan year or a different quantity is not a reliable budget for your prescription. If the response only says “eligible for a coupon,” ask the pharmacy to confirm what you will actually pay before the medication is dispensed.
Does Medicare cover Zepbound for weight management?
Do not use a blanket “Medicare never covers it” rule. The CMS Medicare GLP-1 Bridge runs from July 1, 2026 through December 31, 2027 for eligible beneficiaries. Its $50 copay does not count toward Part D true out-of-pocket costs; the Part D deductible does not apply to this separate route.
