Cigna may cover Zepbound if your plan includes the benefit and its coverage requirements are met. Its published policies describe weight-management and moderate-to-severe obstructive sleep apnea pathways. Your own benefit document, drug list and applicable authorization rules determine the coverage question.
Cigna's policy IP0621 states that weight-loss medications are specifically excluded under many benefit plans, including employer and individual and family plans. Check that exclusion question first. IP0621, page 2.
This guide helps you find the right documents and prepare a coverage conversation. A clinician separately assesses whether Zepbound is medically appropriate for you.
Four checks before you count on coverage
Keep these questions separate when you contact your plan:
Scroll sideways to view all columns.
| Check | What to ask | What the answer does not settle |
|---|---|---|
| Your drug list | Is the exact Zepbound product and presentation on the drug list my plan uses? | Whether your benefit excludes its intended use. |
| Your benefit document | Does my plan include or exclude coverage for this use? | Whether clinical authorization criteria have been met. |
| Authorization | Which policy applies, and what must my prescriber submit? | The amount you will pay at the pharmacy. |
| Your cost estimate | What is my estimated responsibility for this prescription, pharmacy and fill date? | A guaranteed price for a later fill or a different product. |
Use the medication name, presentation and intended use when asking these questions. Keep your member ID and medical records private; the worksheet below is for your own conversation with your plan and clinician.
Find the drug list for your specific Cigna plan
Cigna provides separate drug-list resources for individual and family plans and for employer plans. Its public lists describe medications commonly covered. Cigna says it reserves the right to change this drug list without notice. Choose the list that matches your coverage rather than assuming another person's Cigna list applies to you. Cigna prescription drug lists and coverage.
For an employer plan, Cigna directs members to log in to myCigna and use Price a Medication for their covered medications and specific requirements. Its public employer lookup defaults to a three-tier list unless the list name specifies a tier structure. Cigna directs Small Group members to a separate Small Group Prescription Drug List section because those plans are not yet included in the main lookup. Ask member services to confirm the exact list if you are unsure. Cigna employer-plan drug lookup.
Also check the coverage year and date of service. Cigna's two Zepbound prior authorization documents discussed below, IP0816 and CNF1006, say a coverage determination considers the benefit document in effect on the date of service. A drug list for a later plan year does not establish current coverage. IP0816, page 1, CNF1006, page 1.
What does the $200 monthly-cost headline mean?
In a May 21, 2025 announcement, Evernorth, the health services division of The Cigna Group, described a pharmacy benefit option for Wegovy and Zepbound with patients' monthly costs capped at no more than $200. The announcement, checked October 18, 2026, describes a benefit option, not a current quote for your prescription. Ask your plan administrator whether the option applies to your plan and what current conditions and costs apply. Evernorth announcement, May 21, 2025.
Which Cigna Zepbound prior authorization policy applies?
Prior authorization is a plan review before benefit coverage is approved. It is separate from the clinical decision to prescribe a medicine.
Cigna's policy library separates its drug policies from National Formulary policies. It notes that more than one policy can apply to a benefit, including prior authorization, step therapy or quantity limits. Cigna coverage policy library.
Two published Zepbound documents illustrate why the exact policy matters:
Scroll sideways to view all columns.
| Document | Date shown | Authorization wording in that document |
|---|---|---|
| IP0816, Zepbound Drug Coverage Policy | Effective September 1, 2026 | Prior authorization is required for benefit coverage. |
| CNF1006, Zepbound National Formulary Prior Authorization Policy | Review date June 17, 2026 | Prior authorization is recommended for prescription benefit coverage. |
The two documents in the table, IP0816 and CNF1006, say they apply to health benefit plans administered by Cigna Companies. They also explain that some Cigna companies or lines of business provide utilization review only and do not make coverage decisions. Ask which administrator and policy apply to your pharmacy benefit. IP0816, page 1, CNF1006, page 1.
Both documents contain the same two pathways, one for weight management and one for obstructive sleep apnea. The difference between the documents is not which of those conditions they address. The dates alone do not establish which one governs your plan, and a review date is not an effective date. Ask your plan for the applicable policy number and version. Both documents explain that your particular benefit document takes precedence over the general coverage policy and that applicable laws and regulations must also be considered. IP0816, CNF1006.
What does a weight-management policy look at?
In IP0816 and CNF1006, initial weight-management criteria include age 18 or older, at least three months of behavioral modification and dietary restriction, and baseline body mass index (BMI) of at least 30, or at least 27 with a listed weight-related condition. Initial approval is for eight months; renewal has separate conditions, including at least 5% loss of baseline body weight. Those two documents define baseline measurements as those taken before starting any GLP-1 or GIP/GLP-1 therapy. Ask your prescriber to review the complete policy your plan identifies. IP0816, pages 4-5, CNF1006, pages 5-6.
In the weight-management criteria of IP0816 and CNF1006, the named conditions are hypertension, type 2 diabetes, dyslipidemia, obstructive sleep apnea, cardiovascular disease, knee osteoarthritis, asthma, chronic obstructive pulmonary disease, metabolic dysfunction-associated steatotic liver disease/non-alcoholic fatty liver disease, polycystic ovarian syndrome, and coronary artery disease. This is the list in those two policies, not a diagnosis or personal-eligibility checklist. Prediabetes is not separately named in that list; that fact alone does not establish your coverage or whether treatment is appropriate. Ask your plan which criteria apply and your clinician how your medical history should be documented. IP0816, pages 4-5, CNF1006, pages 5-6.
The BMI examples above summarize selected criteria in IP0816 and CNF1006, not every benefit arrangement. Ask your plan which policies and product presentation apply.
Other Zepbound policies may also apply
Neither a Cigna insurance card nor a policy title confirms that your plan participates in EncircleRx or offers an override. Confirm the applicable policies and product presentation with your plan; do not assume Drug Policy and National Formulary documents apply interchangeably. Three policy families appear below: EncircleRx National Formulary policies, Drug Policy overrides and a quantity limit. Ask your plan which family and policy number it is applying.
Cigna's National Formulary policy index lists separate EncircleRx benefit-exclusion override policies: CNF1011, titled BMI 32 and CNF1012, titled BMI 35. Both state that Zepbound KwikPen is not currently targeted. KwikPen is a single-patient-use pen that holds four doses, according to the Zepbound prescribing information, dosage forms and strengths.
For initial weight management, the baseline BMI branches are at least 32 in CNF1011 or 35 in CNF1012; both also include a branch of at least 27 with at least two listed weight-related conditions present at baseline or currently. Baseline means before any GLP-1 or GIP/GLP-1 therapy. Documentation and other requirements apply. Each has a separate initial sleep apnea pathway with baseline BMI of at least 30 and additional criteria. These offering-specific coverage rules are not clinical treatment thresholds. CNF1011, pages 2-4, CNF1012, pages 2-4.
The separate Drug Policy A-Z directory includes the GLP-1 agonists BMI 32 benefit-exclusion override policy IP0621, effective April 30, 2026 and the Zepbound BMI 35 override policy BEO014, effective September 1, 2026. The same directory also lists a Zepbound BMI 32 override policy, BEO010; this guide does not summarize its criteria. IP0621 calls for lifestyle-vendor enrollment and engagement, with four weigh-ins and four app engagements each month. BEO014 states that Zepbound KwikPen is not currently targeted. Drug Policy A-Z Index, IP0621, pages 1 and 7, BEO014, pages 1-2.
The National Formulary index also lists CNF840, a quantity-management policy; meeting a prior authorization criterion does not settle a quantity restriction. These examples are not a complete policy list.
Does Cigna cover Zepbound for sleep apnea?
IP0816 and CNF1006 each contain a separate pathway for moderate-to-severe obstructive sleep apnea in adults with obesity. Initial one-year approval criteria include a sleep study showing an apnea-hypopnea index (AHI) or respiratory event index (REI) of at least 15 events per hour, baseline BMI of at least 30, and additional requirements. Renewal has different conditions, including completion of at least one year of Zepbound therapy, loss of at least 10% of baseline body weight and stability in sleep apnea signs or symptoms according to the prescriber. The initial-therapy criteria exclude people with central sleep apnea or Cheyne-Stokes respiration. Your clinician must review all requirements. IP0816, pages 5-6, CNF1006, pages 6-7.
The Zepbound prescribing information includes this sleep-apnea indication alongside its adult weight-management indication, with a reduced-calorie diet and increased physical activity. The label does not recommend using Zepbound with another tirzepatide-containing product or any GLP-1 receptor agonist. Insurance criteria are not treatment instructions. A clinician must assess the diagnosis, safety and treatment plan; do not change treatment to match an insurance rule. Zepbound prescribing information, section 1.
The label also carries a boxed warning about thyroid C-cell tumors observed in rats; the relevance to humans is unknown. Zepbound must not be used by people with a personal or family history of medullary thyroid carcinoma; people who have multiple endocrine neoplasia syndrome type 2 (MEN 2); or people with a known serious allergic reaction (hypersensitivity) to tirzepatide or any of its inactive ingredients. The label warns that Zepbound may cause fetal harm during pregnancy. Other warnings and interactions also matter; this is not a complete safety summary. Discuss your history, medicines, pregnancy or pregnancy plans with your clinician. Zepbound prescribing information.
The prescribing information says the sleep apnea studies did not evaluate when, or whether, people who had been using positive airway pressure (PAP) consistently could stop it. Discuss any PAP treatment change with the clinician managing your sleep apnea. Zepbound prescribing information, section 14.2.

If the answer is no, ask what kind of no
Ask for the written reason and the document used to make the decision. Useful questions include:
- Is the issue a benefit exclusion, a drug-list restriction, missing authorization information, or a clinical criterion?
- Which policy number, version and benefit provision explain that reason?
- Is there a review, exception or exclusion-override process under my specific plan? Where are its instructions and any deadlines?
- What information, if any, should my prescriber provide?
An authorization or appeal cannot be assumed to add a benefit your plan excludes. Follow the process your plan identifies, and provide an accurate diagnosis and treatment history.

Frequently Asked Questions
Does every Cigna plan cover Zepbound?
Coverage varies by plan. Check your benefit document, drug list and any applicable authorization requirements.
Does a prescription mean Cigna will pay?
A prescription alone does not confirm that a benefit is included or that any required coverage review is complete. Confirm the plan's requirements and your estimated cost before relying on coverage.
Is a public Cigna drug list enough to know my price?
Use the resource for your exact plan and confirm the specific prescription, pharmacy and fill. A public list of commonly covered medications is not a personalized pharmacy price quote. Also ask whether a plan cost cap or manufacturer savings program applies to that fill and, if so, what conditions must be met.
Does Cigna require prior authorization for Zepbound?
Two prior authorization documents discussed here use different wording. IP0816 says it is required for benefit coverage; CNF1006 says it is recommended for prescription benefit coverage. Ask your plan which authorization and other policies apply to your prescription.
