What happens after the $199 introductory fills?
The standard self-pay price applies after the eligible introductory fills: $349 or $499 per program fill, depending on the pen prescribed. The introductory amount does not apply to every pen. How commercial insurance savings work
The covered-commercial offer requires commercial prescription coverage, a valid prescription for a labeled indication, enrollment and current eligibility. Its “as little as $25” wording is conditional. Maximum manufacturer savings are $100 for a one-month prescription, $200 for two months and $300 for three months; an injection month is defined as 28 days. The card describes eligibility for up to 48 months after enrollment, subject to the manufacturer's ability to change or end the offer. That is not a price lock. The December 31, 2026 deadline above belongs to the introductory offer, rather than the entire card. Savings card terms.
Medicare, Medicaid, VA, Department of Defense, TRICARE and similar government prescription coverage are excluded from the commercial copay offer. Certain other plans and assistance arrangements have additional rules in the full terms. If you are uninsured or have government coverage
The direct pharmacy's eligible self-pay route operates outside insurance. Its terms permit eligible government-insured patients to choose self-pay, so the commercial-card exclusion should not be treated as an exclusion from every cash program. For Ozempic in 2026, NovoCare's patient assistance program sets an income threshold of no more than 200% of the federal poverty level for qualifying uninsured applicants. Citizenship or residency, government-program status and application requirements also apply. Commercially insured patients are ineligible; Medicare beneficiaries with prescription coverage are also ineligible for Ozempic assistance through this program. Why list price is a different number
Manufacturer list price is distinct from a pharmacy cash quote, insured copay or self-pay program price. The official list-price page contained inconsistent amounts when checked October 4, so this guide does not present one as a confirmed current figure. Novo Nordisk has announced a $675 list price effective January 1, 2027. That is a future list-price change, not the October 2026 patient price or a guaranteed future copay.
