Ozempic storage-history worksheet

For a product-specific conversation with your dispensing pharmacist or the manufacturer. Print this page, write down known facts, and mark missing details unknown. If the syringe removal date is unknown, do not invent a new deadline.

Keep this note private. No patient name, diagnosis, address, or contact details are requested. This static page sends no entries anywhere. Record known event dates; do not calculate a discard date on this sheet. The note does not determine whether a medication is usable or provide a dosing plan.

Use the current U.S. directions for your exact presentation. The single-dose prefilled syringe Instructions for Use allow storage out of the refrigerator at 46°F to 86°F (8°C to 30°C) for up to 28 days: record when it leaves refrigeration and discard it if unused within 28 days. Discard a multidose pen 56 days after first use even if medicine remains. Do not use any product past its expiration date. An Ozempic injection that has been frozen must not be used. If storage history is unknown or outside its directions, ask the pharmacist or manufacturer before using the affected product. If an upcoming dose is affected, ask your pharmacist or prescriber for a treatment plan.
